Terms & Conditions

The Low Level Support Service provides information, advice and support to ensure a person’s home is safe, suitable and maintained/adapted to a persons needs including referring to relevant services/providers to enable those interventions to be delivered.  Once a referral has been made a member of staff will contact you within 2 working days

We may refuse a referral if:

  • A person refuses to work with the support workers;
  • A person does not meet the criteria for the Service;
  • A person needs are not eligible or cannot be met by the Service.

After assessment, if deemed not eligible for the Service, you will be informed within 5 days of the initial assessment and will (with consent) be signposted or referred to an appropriate service.
