Who is the service for?

Derbyshire Low Level Support is available to:

  • adults over the age of 18
  • who reside in Derbyshire (excluding Derby City)
  • & who require assistance to maintain their independence in their own homes.

Whether you’re renting privately, own your home, live in a housing association property, or receive support from your local authority, our service is here to help you.

Everyone deserves a safe and happy home. Whether you need help finding a home, or making it safe and accessible, we’re here to lend a hand to support you through a partnership with Citizens Advice Mid Mercia (CAMM), Derbyshire County CouncilSouth Derbyshire CVS (SDCVS) and Revival.  We want to make sure you have a good place to live where you feel comfortable.

Support After Hospital

If you’ve had help from the Home Not Hospital service, we’ll still be here for you afterwards. We want to make sure you have all the support you need to get back on your feet and feel confident at home again.

We welcome referrals from a variety of organisations, professionals, and individuals.

Referrals can be made by:

Healthcare Professionals
Doctors, nurses, therapists, and other healthcare professionals who believe that our support could benefit their patients.

Social Workers or Community Support Workers
Professionals who support adults and families in Derbyshire with various needs.

Housing Associations
Organisations that provide housing services to individuals and families.

Local Authorities
Authorities responsible for providing public services and support within Derbyshire.

Family Members and Caregivers
Relatives or caregivers who are concerned about the well-being of their loved ones and believe they could benefit from our support.

We also accept self-referrals from adults in Derbyshire who believe they could benefit from our services.
